What Is Platelet-Rich Plasma (PRP)?

Platelet-rich plasma (PRP) is a concentrated portion of a patient’s own blood, centrifuged to increase platelet concentration severalfold, then injected into an injured area to help support healing. A clinician might suggest PRP for a stubborn tendon problem or an aching knee. Before agreeing, the more useful question isn’t “does PRP work?” — it’s “what does the evidence say about PRP for my specific problem?”

Blood is mostly liquid plasma, but it also carries platelets — cells best known for clotting that also contain proteins called growth factors involved in healing. To make PRP, a clinician draws a small blood sample and spins it to separate and concentrate platelets, then injects the concentrated plasma into the injured site or, in some cases, applies it during surgery.

Does PRP Actually Work? What the Research Currently Shows

Evidence for PRP varies a lot by condition — it is not a single “does it work” answer. Here’s a condition-by-condition snapshot from current orthopedic research, treated as a starting point for your own conversation, not a final verdict.

  • Chronic tendon injuries: Research shows PRP is effective for certain chronic tendon problems, especially tennis elbow (lateral epicondylitis). Evidence for other chronic tendon injuries, such as long-term Achilles tendon pain, is promising but not yet clearly superior to standard treatment.
  • Knee osteoarthritis: Growing research suggests PRP is effective for mild-to-moderate knee osteoarthritis, with some studies showing benefits lasting up to 2 years. Researchers haven’t yet settled which PRP formulation — leukocyte-rich (higher white blood cell content) or leukocyte-poor (lower white blood cell content) — works best.
  • Acute ligament and muscle injuries: This is the use of PRP that gets the most publicity, often tied to professional athletes, but the research base here is thinner than for chronic tendon problems.
  • Surgical repair (rotator cuff, ACL): Evidence to date shows little to no added benefit of using PRP during rotator cuff or ACL surgery.
  • Fractures: PRP has been tried to help broken bones heal, but research has not shown a significant benefit.

If your clinician recommends PRP for tennis elbow or mild knee arthritis, the evidence base is comparatively strong — a reasonable next step is to ask about the specific protocol. If PRP is recommended as an add-on during ligament surgery, current research shows little additional benefit — a reasonable next step is to ask directly why it’s included and what it’s expected to change.

Is PRP FDA-Approved?

PRP itself is not an FDA-approved drug or treatment for any specific condition. Many clinics use commercial kits — a centrifuge and tube system — to prepare PRP, and clearance for one of those kits confirms the device is comparable to other already-marketed devices at safely separating blood components. That is different from the FDA reviewing or confirming that PRP is effective for a particular condition, so ask your clinician to be specific about exactly what is FDA-cleared: the preparation device, not the treatment outcome.

Separately, the FDA’s consumer guidance on regenerative medicine states plainly that regenerative therapies have not been approved to treat orthopedic conditions such as osteoarthritis, tendonitis, tennis elbow, or knee, hip, neck, or shoulder pain. That notice is aimed broadly at regenerative-medicine marketing, including higher-risk products like stem cells and exosomes, and doesn’t name PRP specifically — but PRP is often marketed under the same “regenerative medicine” umbrella. If a clinic describes PRP as “FDA-approved” for your condition, that claim is worth asking about directly.

Is PRP Safe? Risks and Warning Signs

Reported risks include increased pain at the injection site and — less commonly — infection, tissue damage, or nerve injury, at a rate that appears similar to cortisone injections.

Seek prompt medical attention if, after a PRP injection, you notice worsening pain, redness, warmth, swelling, drainage, or fever at the injection site. These can signal infection or another complication and shouldn’t be ignored until a routine follow-up. For any severe or rapidly worsening symptom, contact your local emergency services.

  • Ask your clinician what symptoms should prompt an urgent call versus routine follow-up.
  • Ask what the expected pain pattern looks like — some soreness in the first one to two weeks is commonly reported before any benefit appears.

What Questions Should I Ask Before Getting PRP?

  1. Confirm the diagnosis and rationale. Ask why PRP is being suggested for your specific condition, not just “for pain.”
  2. Ask about the evidence for that exact condition. Chronic tendon issues and knee arthritis currently have the strongest support — ask how your case compares.
  3. Ask how the PRP will be prepared. Which system or kit is used, and what exactly is FDA-cleared about it?
  4. Ask about alternatives. What would standard treatment, physical therapy, or more time look like instead?
  5. Ask about cost and coverage before scheduling. Confirm directly with your insurer, since coverage for PRP varies and is often limited.
  6. Ask about the recovery timeline. When might you notice any change, and what does follow-up look like?
